Background Check Authorization Statement

I hereby authorize SnubStub Pet Travel and associated companies ("the Company") to obtain a consumer report and/or investigative consumer report about me for purposes of evaluating me as an independent contractor ("Independent Contractor"). This report may include information about my character, general reputation, personal characteristics, mode of living, criminal history, driving record, credit history (if applicable), and other relevant background information.

I understand that the report may be obtained through a third-party consumer reporting agency, and that under the Fair Credit Reporting Act (FCRA), I have the right to request the nature and scope of any investigative consumer report obtained.

I acknowledge that this authorization shall remain in effect for the duration of my relationship with the Company and that additional reports may be obtained at any time, subject to applicable law.

I certify that the information I have provided to the Company is true and complete to the best of my knowledge. I understand that any false or misleading information may result in denial or termination of my contractor relationship.
